Incidents, audits and permits to work
Recorded against the building and the asset, so a risk has a location and a piece of plant attached to it.
Safety records rarely point at the plant they concern.
An incident logged against a site tells you very little. Logged against an asset with its own maintenance and operating history, it tells you whether the plant was already misbehaving.
